What is Continuing Care at Home (CCaH)?

CCaH is a membership-based program that supports older adults to age in place wherever they call home. It usually requires a one-time entrance fee and an ongoing monthly fee in exchange for an initial service package and coverage towards future long-term care services. Individuals join these programs when they are healthy and independent so there is a medical underwriting process for admission. The main service offered in proactive wellness/care coordination that is a holistic, person-centered, and relationship centered approach to assist people age in place through the aging journey to the end of life.

What types of CCaH models exist?

There are different models of CCaH programs that include the traditional Type A model that typically has a larger entrance fee and ongoing monthly service fees and covers long-term care for the life of the member. A hybrid or Type B model that may or may not have an entrance fee but will have an ongoing monthly fee that covers long-term care with certain caps or limits. A care coordination or Type C model that does not cover any long-term care services and solely provides wellness/care coordination and some supportive services. The last model is an Early Advantage model meant for the transition to an independent living dwelling in a Life Plan Community.

How long does it take to implement a CCaH model?

This can be dependent on organization, but we can typically get programs off the ground within 8-12 months.

How many CCaH programs are there within the U.S.?

With all the different models considered we estimate that there are around 52 operating programs. We rely on organizations to self-report what they are offering. We keep a running list of active programs, please contact us if you run a program or similar membership model so we can add you to the directory.
